Overview
  • Developed standardized LOTO posted procedures for heavy industrial equipment across multiple buildings and departments
  • Procedures cover all hazardous energy types — electrical (up to 600 V), pneumatic, and kinetic/stored energy — ensuring zero-energy-state verification before any servicing
  • Each document includes annotated machine photography, colour-coded lockout steps, and a standardized application and removal process
Methodology
  • Conducted hands-on field audits of each machine to identify and photograph all energy isolation points from multiple angles
  • Applied a consistent four-colour lock tag system (Red/Yellow/Blue/Green) across all procedures, aligned with site-wide safety standards
  • Iterated documents through revision cycles with creation and revision dates tracked on each sheet
Outcome
